Starting October 27, 2025, the Geriatric Day Hospital and Geriatric Medicine Clinic will be moving from H-Wing to the Peter Cipriano Centre for Seniors Health in K-Wing.
Located at our Bayview Campus in K-Wing, the Peter Cipriano Centre for Seniors Health is a vital part of Sunnybrook’s vision for inventing the future of health care. Seniors are the fastest growing segment of the population and the highest users of health care in Ontario, and the new Peter Cipriano Centre is dedicated to meeting their unique needs.
The Peter Cipriano Centre features integrated, one-stop facilities, research-inspired care and coordinated access to specialized geriatric services and community resources to help seniors stay healthy and independent for as long as possible, avoiding unnecessary trips to the hospital and the Emergency Department.
The Peter Cipriano Centre for Seniors Health also allows for the collaboration and expansion of the existing geriatric medicine programs in a 13,000 square-foot custom-designed space based on the needs of seniors, inspired by research and consultation with seniors and families.
Patient-centred care
The Peter Cipriano Centre uses a new outpatient model, which begins with an integrated, patient-first approach. Teams in the Peter Cipriano Centre provide comprehensive geriatric assessment and create a personalized plan to manage the care of each patient, coordinating the roles of geriatricians and hospital experts with primary care physicians and community and family supports. The goal is more efficient and compassionate care that keeps seniors independent and in their own homes while reducing health-care costs.
An integrated model of seniors care
The Peter Cipriano Centre takes a new approach to senior’s care, integrating interprofessional teams and services from across Sunnybrook:
- The Comprehensive Seniors’ Care Clinic: This clinic is home to one integrated team of expert clinicians including: geriatricians, nurses, occupational therapists, physiotherapists, OTA/PTA, pharmacist, social workers, dietitian, recreational therapist, and speech language pathology. The new integrated care model ensures care is shared and seamless across the Peter Cipriano Centre team and services are integrated with community partners across the continuum.
- The Outreach pathway: This pathway provides specialized assessments for older adults living in the community, who require in-home service. The goal is to better understand each person’s medical, functional, and social needs through a Comprehensive Geriatric Assessment (CGA). The Peter Cipriano Centre team works closely with family doctors and other health-care providers to make recommendations, connect patients to appropriate services, and support the implementation of care plans. The outreach pathway helps ensure that older adults receive the right care in the right place to support their independence and well-being.
- The Day Hospital pathway: The Geriatric Day Hospital provides short-term rehabilitation and group programs designed for older adults living in the community who are experiencing frailty. It is especially helpful for those who are recovering after a hospital stay or are at risk of hospitalization due to complex health needs. Patients work with the Cipriano interprofessional team to improve their strength, mobility, and independence.
- The Peter Cipriano Centre will also be home to short-term programming including Falls Prevention and Learning the Ropes for MCI (mild cognitive impairment).

Augmentative Services: Augmentative services including Audiology and Optometry, will help support seniors’ hearing and vision health right in the Peter Cipriano Centre for Seniors Health.
